Through a network of local leaders, community organizations, and health systems, Healthy Living for ME® (HL4ME®) delivers programs to help adults manage chronic health conditions, prevent falls, and foster well-being. If you are coping with high blood pressure, heart disease, COPD, arthritis, diabetes, or other chronic conditions, HL4ME can support your efforts to live life as fully and independently as possible. HL4ME’s programs are  proven to make a positive difference in quality of life measures such as feeling more in control, less stressed, and able to do more of what makes you feel well.
Health promotion programs are initiatives aimed at improving health and well-being by encouraging healthy behaviors and creating supportive environments. These programs focus on preventing diseases, reducing health risks, and enhancing the quality of life through education, community engagement, and peer support environments. They address various health issues such as obesity, fall prevention, physical activity, mental health, chronic disease, and pain management, involving community members in planning and implementing person-centered behavior and lifestyle changes.
Many programs offered by HL4ME® partners are evidence-based programs (EBPs). EBPs are interventions scientifically tested and proven effective through research and evaluation. These programs use data to inform their design, implementation, and assessment, ensuring they deliver measurable health benefits. Key characteristics of EBPs include being research-driven, data-informed, replicable, scalable, and cost-effective. Together, health promotion and evidence-based programs help prevent diseases, improve health outcomes, and enhance the overall quality of life for individuals and communities.
HL4ME partners offer programs in person, virtually, by phone, and as independent study kits. A tablet loaning program is also available for participants who wish to join virtual programs but do not possess the technology to do so.

LIVING WELL FOR BETTER HEALTH CLASS:
About the workshop
This six-week class meets once a week for two and a half hours. Each class includes a new topic designed to help participants dealing with chronic conditions. The class is a combination of presentations, discussion, and activities with others who have similar experiences. Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their plan. You will learn about:
  • Dealing with frustration, fatigue, pain, and isolation
  • Exercises for improving strength, flexibility, and endurance
  • Use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • How to evaluate new treatments
A telephone version of the class is also available.
